New CDOT 881 Initial Site Assessment Form!

The Hazardous Materials Group is excited to share that the 881 Form has been updated and modernized to a fillable .pdf with dropdown options, checkboxes, and text boxes. The content is mostly the same with the addition of a "Current or Historic PFAS Use" question under "Potential Environmental Concerns" and an updated Signature line stating, "Was this assessment completed by a qualified Environmental Professional (EP) as defined by the Environmental Protection Agency All Appropriate Inquiries Rule (40 CFR Part 312)?"  Please download and use this new version moving forward. We hope you enjoy and appreciate this new version.
